@Don... I am pretty sure that the rebate form you posted came from the CC site not rebate HQ... I never did find anything at rebatehq saying the rebate was valid.

Might not be useful now, but I think people might find it helpful as rebates begin getting denied and come back to check this thread.
what the shell! i called CC 2x and they kept saying this is a manufacture rebate, please give the manufacture a call.
After you have a rebate tracking number (best obtained quickly by using fax 888-864-5763, otherwise obtained (slowly) by mailing as on the receipt)
When you go to the rebatehq website and put in the tracking number you will discover that your rebate will be invalidated because of:
Dates not eligible
Invalid UPC
Just call Parago Escalation ("the manufacturer") directly:
877-681-2362
When you speak to the representative he wil robotically repeat the same thing:
"Our system shows this rebate ended for this offer"
Do not accept this.
Politely and firmly tell him you have copies of the documentation of the receipt. Read the receipt to him. Offer to email him the receipt as a jpg to an email. Do not accept a fax number, the escalation department never sees those, they go somewhere else entirely.
As for the UPC? Tell him there is only one UPC on the box. Offer to send him a jpg of all six sides of the box. Tell him there is a serial number on the card and offer to scan it and send it to him.
Don't worry if you don't have a scanner. Doesn't matter. They don't want you to have their email address anyway.
Basically you need to make approving the rebate easier than verifying your claims. They can and will approve your rebate unilaterally just so you will go away. Takes 2-3 minutes. It's not hard, after all: YOU ARE IN THE RIGHT.
(Some people will reasonably say that this does not address the larger issue for everyone else. My Response? "Yup, you are right!...aaaaand thank you for volunteering!"

If for some reason the guy will not approve your rebate, ask him to contact cc corp and conference in a representative. If he won't, hang up, contact cc and have them conference him in. (It never came to this for me, not even close, but it's the logical next step.)
I have helped three friends who got in on the deal and each time this procedure worked. (I actually made the call each time because I was the one to tip them to the deal and felt responsible.)
Just for the record if you prefer not to follow this route: take the Rebate HQ invalid upc info to the store where you bought it along with the rebate, ask to speak to the store manager and get them to credit the rebate via price-adjustment. One friend of mine did this instead and it also worked.
Don't stress, it really is easy.
