Don't know for sure of course, but my first guess is "incompetence".

Forgot why I pulled this up, but the official stats for hospital deaths due to "mistakes" was something like 98,000 PER YEAR, only in the US. Then a couple other studies came out that bumped it to 210,000 and 440,000.

That's a freaking lot of people even if the low end is true.

Well, it's the misleading claim as to what happened to the poor woman, that caused her family so much unnecessary grief, that's the issue. By "something darker" I don't mean to imply murder, but just whether the miscategorisation of what happened was purely administrative ineptitude (quite bad enough in itself) or if it was somehow related to a reluctance to investigate or acknowledge mistakes in her treatment.
