Contributed to a system crash earlier this year, I was attempting to help someone at another site cancel a Race, when they didnt seem able to I went in and did it myself. Aparently our software doesnt like 2 people doing the same command for the same thing at the same time, and slowly locked up the entire system.... that night was hell........

Miss quoted a mortgage rate by .250% because of a computer error......

I felt like a complete ass, but all my boss had to say was that while he wanted me to be more careful, I close almost 200 of these things a month and the slip up was like a rain drop in a lake.........

Meh, I beat myself up mostly.

Banged a cute little Korean girl I hired to work for me in the office...on my bosses desk. That was years ago though and nobody found out. Well, we kept a relationship for about a month and a half but then she moved back up to the Bay area.

Lately, probably just spend too much time on ATOT.

Hmmm - perhaps when I was a bit tired and not particularly paying attention one summer. I work as a groundskeeper, and I almost ran into my boss while driving a John Deere Gator and rounding a corner. He was driving a skid loader with a pallet full of something heavy. A head on collision would not have been good, but luckily I swerved just in time. He was a tad bit pissed.

I used to work in a deli...and we have a register for deli business and another for lottery business.

One day a guy came in to cash a scratch off..

thinking that it'd only be a couple dollars..i didn't check the pay off before I entered in the code.

turns out he won 1000 dollars. It was after the boss's hours...and there wasn't enough money in the lottery register...so i had to apologize to the dude..call boss at home...have him come in for emergency cash reserve to give the dude his 1000. Yea..it was pretty bad. I got into so much trouble for it.

Most recently, I discovered through a stupid error, an undocumented flaw in some of our support software. The flaw, it's discovery and resolution, caused 1700 retail locations to NOT be able to exchange data with the corporate side for two days and caused my boss to put in about 15 hours of overtime between the two days. Essnetially I could have avoided the whole mess if I had more carefully reviewed to short text documents, to make sure they were what I needed to put in place.

Before that, in my previous life, I would occasionally mismeasure the salt and have to throw out 5-600 pounds of dough.

disconnected the accessory gearbox driveshaft instead of the APP driveshaft like i was supposed to adding about oh 2 hours of work in the cold. =\

Hiring a girl I wanted to date without knowing her THAT well. Dated her, got to know her. Broke up with her, and still have to work with her.

She is so damn annoying, I can't stand it.

Back in high school, I worked in a restaurant. One particularly annoying waitress was having a stellar night at pissing me off. She did it one too many times and I beaned her in the head with a head of lettuce. (Thankfully, that's what I had in my hands at that moment!) Didn't even get fired, since the boss knew she deserved it, although he wouldn't admit it to anyone else. I gave my two weeks notice the next morning.

When I was 14 I was doing street maintanance--tarring open cracks on city streets--and I filled a bunch of cracks around a manhole, but the tar ran down so it ended up sealing the manhole down.

Hmm... I once configured the Novell network settings incorrectly on one of the operating system builds that I made for the university IT department. That OS build got deployed to 30 systems, and generated enough garbage network traffic to bring down the entire campus file and print sharing network daily for a week until someone finally figured out that the problem was. Oops.

Thank you for saying beaned... I can't stand people who say they "beamed" someone in the head...

Probably I just was the reason something got sold for a little more than we paid for it (typo or otherwise) a couple times in the past couple years.

I failed to remove a plastic cover from a UV laser power sensor. When we fired up the laser (7W) a bit of the vaporized plastic was deposited on a $7500 lens, destroying it.
