Baseball. I remember reading an article about Ted Williams and his eyesight was off the charts. They contribute his ability to hit greatly to his eyesight. He could actually see the stitches of the ball and how it was spinning.
A lot of other sports require good eyesight, but it's almost more of a function of depth perception and recognition of target. I've got horrible vision and without glasses I can barely see the net of a basketball hoop but still can easily hit shots that far out.
