When everyday is a struggle to get up because you hate your job so much. When the stress of how miserable your are at your job is so consuming, that it substantially affects your life outside of work.
I was there a couple years ago. Finally got up the backbone to quit and change careers, after much encourage from my wife. I'm sooo much happier for it.
In your case, I'd quit. Having panic attacks over your job just isn't worth it. Change careers, go back to school...whatever. Even if you lower your standard of living through a lower paying job that you enjoy, you'll be happier. Sell your house (if you have one) and move in to an apt if you have to.
I imagine such a dilemma is much more horrific for people with kids and a standard of living that they *must* maintain.
