Where to obtain my latest medical history?

I don't have a personal doctor that I go to and I have not been to the hospital in years. Would my undergraduate college from nearly a decade ago be the best place to ask?

Seriously though. It sounds like you need a complete physical to start a medical history.

Your mom. Seriously... she would have your immunization records. I would think that would be the only medical history one would need. But as others have said if this is not what is being asked of you...then you do not have a medical history.

Is this for a job or buying insurance?

Most hospitals keep inactive medical records for the statute of limitations, unless there is a state law that specifies otherwise. 15 years + however long until the records are thinned. You contact the care provider (hospital) to obtain your medical records and complete a request for information.
