NOT the southeast.
edit: opposite of southeast sounds good.

I like cool weather, and not many people. If I were to move out of MD, I'd go to Washington, or Maine. Washington would probably have more fun things for a young guy, Maine is more laid back, without as much to do.

I have lived in the Midwest and the Southwest, and much prefer the southwest.
In the midwest, I lived in Columbus, Ohio, which is a nice city and still has lots of work last I asked my mom, but the winters there are too depression inducing. I now live near Santa Fe, New Mexico, and the jobs are very scarce except in Albuquerque and Las Cruces. The weather here is many times better than columbus, but there is still a short growing season.
What kind of work are you looking for?

I'll be working in radiation oncology. I am planning on picking where I go by going where the jobs take me. I can also do travel work (13 weeks one place, 13 in another, etc).

What kind of "culture" do you prefer?
For me, nothing east of the Rockies would suffice.
I prefer the laid-back attitudes out west over the "hurry, hurry" ways that you find back east, especially in the cities...(north east, not south east)
It sounds like your profession should give you lots of options.

I prefer the cold and snowy weather of the northeast, but the red state culture of the southeast. Help.

I live in Burlington VT now... it's got a record for being ultra-liberal and it is, but if you get more then 25 miles from the "city" it's like being in the deep south... it's much like way way upstate NY in this regards.

If you have the option I'd do the travel thing for a year or so and move around the country. You'd get a great way to see the country and decide where you want to live.
