Worst screw-up at work

I had a nice one yesterday.

Attempting to remove an OSPF neighbor from a 6506 router.

Pasted line one
router ospf 100

Meant to paste line two
No network 10.228.0.0 0.0.1.255 area 0

Actually pasted
No router ospf 100

DOH!

63 customer sites now unreachable.

RELOAD DAMNIT RELOAD!!!

I had typed 'no' and thought I pasted the network but router ospf 100 was still on the clipboard.

I ran a delete query on a table with 100000+ records.

I forgot my where clause.

Authorized a $5,000,000 policy when it should have only been a $1,000,000 policy. So, the company got an extra $4,000,000 of coverage for 12 months. Had a nasty claim come in, I would have been looking for a new job. I got lucky on that one.

I once cancelled an OC3 line between us and Verizon, which was our P2P for our work on the FIOS project.

I once OD on PAS at work causing a QWA.

I almost got fired for it.

Wasn't mine, but I was present. Fellow ICman was closing the hangar bay door on a destroyer (DD-980) without inserting the track for the helo thingy (can't remember what it's called, but it has the cable that drags the helo to the deck). We had pretty much the entire ships crew with ropes & cables to lift the door back onto it's track--got it too after a while.

Back when I worked in a kitchen ( I was about 19 years old):

I spent my whole morning preparing the feature dish for the day. Portioning all of the required ingredients into different little cups and stacking them all neatly on three different trays. About 120 portions all together.

Of course, I only wanted to make one trip to the walk-in cooler (time is money after all), so I stacked the trays on top of each other. As I round the corner of the kitchen, I stumble and start to fall, and the trays ended up launching against the wall.

There were onions, sundried tomatoes, capers, oil, garlic, etc, everywhere. Of course I also fell and landed right on all of them creating an even bigger disaster (and bruising my ego further).

We had to remove the feature from the menu for the day.

That was much worse than when I cut the end of my finger off.

I dumped about a ton of molten iron on the floor of the foundy i worked at for a summer job.

I ment to lift the ladle up rather then make it rotate... whoops. It kinda looked like a 2,000 pounds sparkler. Luckly the setup is designed for mistakes like that so one one got hurt, but did i ever feel like a jackass.

The clean-up guys gave me some pretty evil looks, too.

Aside from accepting the job offer as someone just mentioned, several years ago I was solely responsible for toppling over a brand new 20,' $200,000 panel saw crashing it to the ground, the week it was scheduled to be shipped to the customer who just purchased it. Thank God for insurance but that was NOT a good day. The whole thing happened in slow motion and I just put my head down, nearly crying.

Hit my boss in the head with one of those plastic tubs. I was pulling it down from a shelf and right as he stood up BAM. Almost got fired, but oh well.

Crashed our system software while trying to help someone at a remote location. Apparently it didn't like two people accessing the same module at the same time. Something that has been fixed in later versions because of it.

Wasn't mine, but office mate re-ordered our pyramids for out ARCGIS images (different images at different altitudes to make sampling faster) and didn't perform his test first. Deleted our entire image database and failed to load the new ones, so we had nothing. We had to get our backups loaded and took about a week, cost about a million dollars in total...

They took away his update privileges on all databases
